If you have not already done so, please review the opportunities offered in our online prospectus prior to filling out this order form.

Your organization can choose to be an exhibitor only, sponsor only or sponsor with a booth.

To reach a sponsorship level, you can:


1. Design your own sponsorship package by choosing items from the options shown in the upcoming tabs (booths, advertising opportunities, speaking opportunities, brand exposure, interactive activities and more or;

2. Make a general contribution to the Congress by selecting only the sponsorship level or;

3. Choose a combination of items and general contribution

If you previously completed the "Express of Interet Form", we'll keep your interested priority based on when you completed the express form but we'll consider the symposium type selected in this order form. Please note that the date/location will be based on sponsorship level - first come first serve basis. Ex: Platinum sponsors selected the morning symposium, the first one to select the date will be the first one that completed the express of interest form.

Please indicate your method of payment. For payments made by credit card, a 4% fee will be applied.

Summary

Reminder: the funds in your selected sponsorship level can be used to select any opportunities available. 

Example:
Company A: Selected an inline booth with exhibitor package 2 (USD 6,500). They will be recognized as an exhibitor.

Company B: Selected a Peninsula booth (USD11,000) + Push Notification (USD 3,500). Company B will be recognized as a Bronze sponsor and will receive all the benefits within this level.

Company C: Selected an Industry Satellite Symposium (USD 110,000) + general contribution (USD 40,000) to reach the gold level. Company C will be recognized as a Gold sponsor and will receive all the benefits within this level (including being the 3rd group to select the Satellite date/time).

Company D: Selected an Industry Satellite Symposium (USD 110,00) + Hotel Key Cards (USD 30,000) + City advertisement ad (USD 50,000) + 30’X30’ booth (USD 38,700) + Cardboard lounge (USD 45,000). Company C will be recognized as a Platinum sponsor and will receive all the benefits within this level.
